Shannon's School of Dance 2009-2010 Registration Form
Classes will start the week of October 5th!!!
Cosmetically we are not ready....however, we are still starting classes! Please keep in mind that we still need to replace a ceiling , paint etc...so pardon the decor!!! :) If you have not yet registered, we prefer you do so prior to coming to class in order to minimize the confusion..It is hard to put a class roster together when we don't know your child will be there! If you have paid your first payment....your next payment will not be due until November, however your installment fee will be slightly higher, due to the fact that we have to reconfigure payment plans because of the delay. Classes will be guaranteed, however yearly fee will be broken down into 9 payments instead of 10- this will make your payment per month slightly higher ( around $5) because we are spreading out one payment over nine months. We did not think that it was fair to ask for two months of payments when we are just starting classes,so we surveyed some parents and came up with this solution.
We didn't anticipate the tremendous delay, and look forward to starting classes! :)

Registration Fee: $35.00 for Individual Student, $45.00 for 2 or more children
(returning students take off $10 if fees are sent in prior to September 10th)
After September 10th, returning student registrations will be $35/$45
Returning Students: September installment and registration fees are due by September 10th in order to apply a $10 discount to September.
Classes begin the week of September 15th, 2008
Registration fee and first month's tuition is due at the time of registration.
Now accepting Visa and Mastercard.
Payments October thru June must be accompanied with the appropriate payment coupon from the payment coupon book.
Tuition installments will be due the first week of every month, regardless of number of classes that meet in that month. Tuition is a yearly fee, broken down into 10 payments (September thru June) for your convenience.
A $15 late fee is charge to all accounts not current on the 11th of each month that is past due. Any and all payments received after the 10th are considered LATE.
! Automatic payments may be charged to your credit/debit card for monthly or quarterly payments.
If you are interested in setting this up with an additional $1 per month fee, please let us know!

Please note: Returning students only need to fill out their name and note the class(es) (unless address, email and phone information has changed) they are interested in taking.
For students who have not been enrolled in the past two years, we need all information filled out.-Thank You!
